To permanently delete your JAMS login account, including your login credentials and app authentication email address, fill in the information below.

Your JAMS login account will be deleted within 5 business days.
You will receive a confirmation of completion at the email address you provide. Your SMW Local 104 membership will not be terminated, and your membership records will not be deleted from Local 104’s systems. If an email address is stored separately in your membership record, that address may remain as contact information but it will no longer work as an app login.

To resign your membership, or obtain a withdrawal or transfer card, contact your local dispatch office.
